Rename ModuleToIndex to PalletRuntimeSetup ( #7148 )

* Rename `ModuleToIndex` to `PalletRuntimeSetup`
Besides the renaming it also adds support getting the name of a pallet
as configured in the runtime.
* Rename it to `PalletInfo`
* Remove accidentally added files

Extract frame_system SignedExtensions into separate files. ( #6474 )

* Rename CheckEra -> CheckMortality but keep backwards compatibility
* Update frame/system/src/extensions/check_mortality.rs
* Don't rename the IDENTIFIER for now.
